Combinations of variations and attribute values

Hi
I am just going to define a page for print order , something similar to what you provided
at business card print in
http://themecomplete.com/api/preview/?product=epo
I defined 2 different attributes as variable for product "Leaflet & Flyer"

1- Paper thickness:
An attribute with different selection (135 g/m²,170 g/m²,300 g/m²,400 g/m²)
this one assigned as variantion for this Product
such as
#3891---> 135 g/m²
#3890---> 170 g/m²
#3889---> 300 g/m²
#3888---> 400 g/m²

2- Paper Run :
Attribute which is not used for variation (25,50,100,250,500,1000,1250,2000,5000)
The values of that attribute will be used to create the extra option.

I used local mode at "TM Extra Product Options" panel
then by click on "Add Extra Option" button
I add 1 Attribute as extra option , for my intendtion - I choose it as Type << select >>
and assigned to each above Variation (#3891,#3890,#3889,#3888) the appropriate attribute (25,50,100,250,500
with exact price as fixed amount

first question:
As you can see Unfortunately extra options are not shown up on the example shop page

second question:
all variation from #3791 to #3788 and similar do not need a base price,
at our main shop their price is related to the selected quantity, but if I want to show them I have to add a price in variation section otherwise, they were not shown up
just for checking
when I assigned price €12.00 for variation
#3891---> 135 g/m² and
#3890---> 170 g/m²
they appear in front shop in front of "Paper thickness"

how I can add those variations without any base price (no Price) then users select price via other selection options.

would you please let me know what I have missed?
