Extra Product Options not showing on front end

    Extra Product Options not showing on front end for admin, non-admin, and non-logged in. Tried various settings on "Enable Front-End For Roles", including selecting no role, all roles, everyone, etc. I've used the plug-in successfully on another site, purchased a new license today for this site I'm working on now.

    I tried to submit this under "Global Product Options" but my account logs out every time I navigate to that portion of the forum.
