Text fields not saving

Hi,

I have some gift options on the checkout of a site. There are 3 text input fields that don't seem to be saving the data entered into the field. Sometimes its only the first one. I can't see any errors but if you could have a look.

I've attached a screenshot of the text fields that seem to fail to save on https://bathhouse.sqsite.co.uk/checkout/
